  • Name, phone number, and email address of contact person reporting issue
  • Which Component, Device, or Softphone is having the issue
  • The User Logins assigned to the affected user(s)
  • Number of End-Users affected
  • How long the issue has been occurring if the issue is constant or sporadic
  • Clear, detailed description of the issue, including the specific text of any error messages
